Key Responsibilities
- Provide occupational therapy evaluations and treatments to home health patients in the Concord and Walnut Creek areas
- Actively manage your caseload to maintain expected productivity targets, including proactively backfilling cancelled visits
- Document patient care accurately and in a timely manner using Home Care Home Base (HCHB)
- Coordinate with the home office team to ensure continuity of care and optimal scheduling
- Track and report mileage in accordance with billing guidelines
Required Qualifications
- Active Occupational Therapist (OT) license in the state of California
- Minimum 1–2 years of occupational therapy experience required
- Home health experience required
- Proficiency with or ability to learn HCHB (Home Care Home Base) documentation software
- Valid driver's license and reliable transportation for patient home visits
Preferred Experience / Skills
- Prior experience with HCHB documentation platform
- Demonstrated ability to independently manage a home health caseload and maintain productivity
- Familiarity with the Concord/Walnut Creek geographic area
