Key Responsibilities

• Evaluate and treat residents within a skilled nursing facility setting to restore or maintain functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient goals and clinical findings
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to coordinate comprehensive resident care
• Educate residents and family members on therapeutic techniques and home strategies to support continued progress

Required Qualifications

• Current and valid Occupational Therapist (OT) licensure in the state of Colorado
• Graduate of an accredited Occupational Therapy program
• NBCOT certification (OTR) required or eligible
• Experience or willingness to work in a skilled nursing facility environment

Preferred Experience / Skills

• Prior experience working in a SNF or long-term care setting
• Familiarity with MDS documentation and Medicare/Medicaid compliance requirements
• Strong interpersonal and communication skills for working within a multidisciplinary team
