    Key Responsibilities
    • Assess patients' level of functioning upon physician orders and plan, implement, and adjust individualized treatment plans based on evaluation results and patient/family goals
    • Participate in the interdisciplinary team process, including team conferences, and collaborate with other team members to provide comprehensive patient care
    • Coordinate discharge planning for each patient, including placement recommendations, patient and family education, and adaptive equipment issuance and training
    • Complete all required documentation including initial evaluations, daily progress notes, treatment plans, and discharge summaries in accordance with department policy and defined timeframes
    • Supervise Certified Occupational Therapy Assistants (COTAs) and OT/COTA students in the application of occupational therapy theory and techniques; assist with new staff orientation
    • Promote patient and employee safety through safe patient handling, infection control practices, and adherence to all facility safety policies and procedures
    Required Qualifications
    • Current Occupational Therapist licensure in the state of Ohio
    • Ability to plan, organize, and administer occupational therapy treatment in accordance with applicable federal, state, and local standards, regulations, and guidelines
    • Demonstrated ability to supervise therapy assistants and students competently
    • Strong documentation skills with the ability to complete evaluations, progress notes, and discharge summaries within defined timeframes
    Preferred Experience / Skills
    • Experience participating in interdisciplinary team conferences and collaborative care settings
    • Familiarity with discharge planning processes including adaptive equipment recommendations and caregiver training
    • Demonstrated customer service orientation with patients, families, and interdisciplinary team members
