Key Responsibilities of the Occupational Therapist- Provide occupational therapy services focusing on ADLs and adaptive equipment
- Conduct evaluations and create individualized treatment plans for home health patients
- Collaborate with RNs and PTs to deliver coordinated patient care
- Participate in daily morning team huddles via phone
- Manage a case load of up to 6 patients/points per day
Requirements of the Occupational Therapist- Current WI License and BLS certification
- Minimum 3 years of OT experience, preferably in home health
- Bachelor's degree in occupational therapy
- Valid driver's license and active auto insurance
