Job Opportunity – Occupational Therapist, Home Health – Medford, OR 97504
Start Date: September 21, 2026
Location: Medford, Oregon and surrounding areas
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. One-on-one patient care with up to 6 patient visits per day. 30 productivity points standard. No weekend requirement. No on-call requirement.

Requirements:- Active Oregon Occupational Therapy license (compact license accepted; must be in hand at submission)
- Minimum 1 year of skilled home health experience required
- BCLS (Basic Cardiac Life Support) certification required
- Oasis documentation experience required
- COVID-19 primary vaccination series plus one booster required
- Reliable personal transportation required for home visits
- Two professional references covering 1 year within the last 3 years
