Description:
Effectively screens and evaluates patients with cognitive and/or activities of daily living (ADL) physical functioning disorders.
Develops an appropriate plan of care following all regulatory and clinical practice standards.
Utilizes standardized assessments and evidence-based practice to support clinical interventions.
Provides rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders.
Administers various procedures as part of the rehabilitation plan, which may include ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
Assesses patient progress and adjusts treatments accordingly, documenting in the clinical record per regulatory and clinical practice requirements.
Evaluates treatment outcomes for effectiveness, modifying services as necessary based on patient evaluation and making referrals when appropriate.
Provides consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
Provides comprehensive discharge summaries of services provided according to regulatory and clinical requirements.
Documents all regulatory and clinically required information in the electronic documentation system.
Maintains patient records in accordance with company policy.
Records treatment minutes accurately for services provided per regulatory and clinical requirements.
Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings.
Participates in facility and company-required trainings, in-services, and conferences.
Able to obtain necessary durable medical equipment or supplies per regulatory and clinical practice requirements.
Supervises and supports occupational therapist assistants, rehabilitation aides, and/or students as required.
Adheres to all relevant practice acts, codes of conduct, and ethics during professional practice as an occupational therapist.
