Description:
Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders.
Develops appropriate plans of care following regulatory and clinical practice standards.
Utilizes standardized assessments and evidence-based practice to support clinical interventions.
Provides rehabilitation, skilled, and medically necessary treatment interventions to patients.
Administers various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, and modalities.
Uses supportive and assistive devices.
Assesses patient progress and adjusts treatments accordingly while documenting in clinical records.
Evaluates treatment outcomes for effectiveness.
Modifies services in relation to patient evaluations and makes referrals as needed.
Provides consultation and counseling to patients, families, caregivers, and other service providers.
Effectively communicates with supervisors and other health team members regarding patient progress and treatment plans.
Provides comprehensive discharge summaries of services per regulatory and clinical requirements.
Documents all required information in electronic documentation systems.
Maintains patient records according to company policy.
Accurately records treatment minutes for services provided as per regulatory requirements.
Participates in required meetings including patient care conferences and rehabilitation meetings.
Engages in facility and company required trainings and in-services.
Obtains necessary durable medical equipment or supplies as required.
Supervises and supports occupational therapist assistants and rehabilitation aides.
Adheres to required practice acts, code of conduct, and ethics during professional practice.
