Key Responsibilities
Provide occupational therapy services to patients in both Skilled Nursing Facility (SNF) and Home Health settings
Meet productivity expectations of 87% in the SNF setting and 95% in the Home Health setting
Utilize Net Health and Home Care Home Base software for documentation and patient management
Deliver patient care under the supervision of an on-site Therapy Supervisor
Follow established care plans and contribute to patient progress toward functional goals
Required Qualifications
Active COTA licensure or certification in the state of Minnesota (or eligibility to obtain)
Must reside within 80 miles of the facility to be eligible for this position
Ability to work across SNF and Home Health care settings
Proficiency or ability to learn electronic documentation platforms
Preferred Experience / Skills
New graduates are welcome to apply
Prior experience in SNF or Home Health settings is a plus
Familiarity with Net Health and/or Home Care Home Base documentation software
