Responsibilities
- Evaluate patients and develop individualized treatment plans
- Assess functional limitations and implement evidence-based therapeutic interventions
- Help patients improve activities of daily living, mobility, and self-care abilities
- Monitor patient progress and adjust treatment plans as needed
- Educate patients, families, and caregivers on therapy techniques and home exercise programs
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
