  • Preferred Qualifications: OASIS experience required for SOC staff, familiarity with home health documentation systems.
  • Experience Requirements: Minimum of 1 year of relevant experience with adult patients in home health, no inpatient experience accepted.
  • Special Requirements: Must have own transportation, active drivers license, and be prepared for potential on-call rotation; timely communication and documentation are mandatory.
