Job Details
- Position: Occupational Therapist for Home Health (OT HH)
- Location: Missoula, MT
- Schedule: Days, 8-hour shifts, 40 hours per week
- Required Qualifications: BS degree or higher from an accredited Occupational Therapy program, current license to practice in the State of Montana, valid drivers license, appropriate auto insurance, 2 years of previous OT experience preferred.
- Additional Information:
- Evaluates and establishes care plans tailored to patient needs in the home environment.
- Coordinates with other disciplines and seeks further orders as necessary.
- Provides direct patient care based on the established care plan and physician directives.
- Applies knowledge of evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in assessments and treatments.
- Modifies treatment plans, including frequency and duration, as appropriate.
- Masters scoring of the CMS required OASIS and collaborates with the Quality team for adjustments.
- Participates in multidisciplinary conferences and communicates effectively with physicians and team members.
- Documents all services, adverse events, and communications in the medical record.
- Engages in Quality Assurance Performance Improvement initiatives.
- Ensures compliance with licensure and continuing education standards.
- Supervises Occupational Therapy Assistants and performs supervisory visits within required timelines.
- Manages care coordination, including admissions, discharges, and OASIS timing.
- Participates in agency-provided educational opportunities and performs other duties as assigned by the supervisor.
