Job Summary:
Occupational Therapist Home Health (OT HH) position located in the designated service area. This role involves providing home health occupational therapy services to geriatric patients, with a focus on timely documentation, communication, and patient care. The assignment requires flexibility within the territory, adherence to clinical standards, and maintaining appropriate certifications.

Job Details:

  • Schedule: 8 hours per day, minimum of 40 hours per week, starting 10/05/2026 and ending 01/04/2027.
  • Required Qualifications: Valid license in hand, minimum of 2 years of relevant experience including OASIS experience for staff completing Start of Care, proficiency in HCHB (Homecare Homebase), current BLS certification, active driver's license, and personal transportation.
  • Preferred Qualifications: Experience with home health, prior hospice or community hospital experience preferred, familiarity with complex patient care involving geriatrics, and experience with interdisciplinary support.
  • Additional Information:
    • Home health occupational therapist responsible for geriatric patient assessments and therapy plans.
    • On-call rotation may be required; details to be discussed during interview.
    • All travelers must have an active drivers license, personal transportation, and RTO (Requested Time Off) must be submitted at application.
    • Modules are billable and completed during orientation.
