Start Date: September 7, 2026

Location: Medford, Oregon and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), 5 days per week. This position does not accommodate 4x10 or 3x12 schedules. Typical caseload includes up to 6 patient visits per day with a standard productivity expectation of 30 points. No weekend or on-call requirements.

Requirements:

• Active Oregon Occupational Therapy license (pending licenses accepted)
• Minimum 1 year of skilled home health experience required
• BCLS (Basic Cardiac Life Support) certification required
• Oasis documentation experience required
• Reliable personal transportation is essential for home visits
• COVID-19 primary vaccination series plus one booster required
• Two professional references covering 1 year within the last 3 years
• Willingness to travel within service area (occasional radius of 50+ miles)
