Occupational Therapist (OT) – Home Health

Key Responsibilities

  • Provide occupational therapy services to patients in a home health setting within a 50-mile radius of the office
  • Maintain expected productivity of 30 points per week (6 points daily)
  • Conduct evaluations, develop treatment plans, and deliver therapeutic interventions aligned with patient goals
  • Document patient progress and clinical notes utilizing HCHB (Homecare Homebase) software
  • Collaborate with interdisciplinary team members to coordinate patient care

Required Qualifications

  • Active Occupational Therapist (OT) license in the state of Oregon
  • Previous home health experience required
  • Previous HCHB (Homecare Homebase) experience required
  • Ability to travel within a 50-mile radius of the office

Preferred Experience / Skills

  • Strong time management and self-directed work skills suited for a home-based caseload
  • Experience managing productivity standards in a home health environment
