Key Responsibilities

• Conduct skilled home health visits for a diverse patient population including med/surg, cardiac, ortho, post-surgical, COPD, CHF, CVA, neurological, and trauma cases, among others
• Perform and document OASIS assessments in accordance with Medicare, Medicare Advantage, and commercial insurance requirements
• Complete a minimum of 6 skilled home visits per day, with potential for additional visits based on manager discretion
• Self-schedule visits and hours based on the needs of the home care office, including 1–2 weekends per contract period
• Utilize facility-provided laptop, MiFi, and portable printer for documentation using designated scheduling and timesheet systems
• Assist patients with ADLs and therapeutic interventions targeting functional recovery and independence

Required Qualifications

• Active Florida or Compact Occupational Therapist license required at time of submission
• BLS certification required
• Minimum of 1 year of occupational therapy experience required
• Home health experience required
• Valid driver's license and reliable personal vehicle required for home visits
• COVID-19 vaccination required, or exemption form on file; flu shot or declination required with mandatory mask wearing if declined
• Must reside more than 100 miles from the facility to be eligible

Preferred Experience / Skills

• OASIS training or prior OASIS documentation experience preferred
• Cerner EMR experience preferred
• Experience working with neurological, orthopedic, and post-surgical patient populations
