  • Required Qualifications: Two years of previous OT experience; Iowa state licensure; BLS certification; valid driver's license; ability to work within a 50-mile radius of Fort Dodge, IA; experience with electronic medical records (Epic experience preferred); work history/resume required; skills checklist completed within 1 year of submission; copy of certifications and license verification required; referencesone supervisory reference required at time of submission.
  • Preferred Qualifications: Home health experience preferred but not necessary; Epic experience preferred.
  • Additional Information: The assignment involves working with a small team that includes a physical therapist, occupational therapy assistant, and certified occupational therapy assistant. Orientation includes general organization training, Epic EMR training, and shadow visits, with full productivity expected by week 3. The role requires travel within a 50-mile radius, with mileage reimbursement. Clinicians must have access to own vehicle. The position involves working 1-2 holidays (Memorial Day, Independence Day, Labor Day).
