Facility Name: Ottumwa Regional Health Center
• Facility Department/Cost Center: Ottumwa Regional Health Center/ ARU
• Role (RN, Tech, LPN, PT, etc.): OT
• Shift: Day
• Weekly Guaranteed Hours: 32 hours
• Weekend Hours: Will work at minimum 1 weekend per month
• Call Requirements: none
• Required Certifications: Masters from accredited school/ Licensed in state of Iowa
• Years of Experience: 0- 1 year
• Float Requirements: Yes, will be floating from ARU to MS to cover evaluations and treatments as needed.
• Number of Beds: ARU 13 beds/ MS 24 beds
• EHR: Net health documentation
• Patient Population e.g. “geriatric, adult, peds, etc.”: Geriatric, adult, general diagnosis
• Required Education: Accredited degree from OT school

• Scrub Color: Any color
• Any unacceptable attire: Pajamas and torn clothing
