Occupational Therapist

Required Qualifications

  • OT Experience: 1 year required
  • LTC or Rehab Experience: Required
  • OT License: Required
  • Must follow NBCOT/AOTA and applicable state practice acts, code of conduct, and ethics requirements.

Key Responsibilities

  • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders.
  • Develop appropriate plans of care.
  • Provide skilled, medically necessary rehabilitation treatment.
  • Use standardized assessments and evidence-based practices.
  • Perform:
    • ADL retraining
    • Therapeutic activities
    • Therapeutic exercises
    • Modalities
    • Supportive and assistive device training
  • Assess patient progress and modify treatment plans.
  • Evaluate treatment outcomes and make referrals as needed.
  • Provide consultation and counseling to patients, families, caregivers, and other providers.
  • Communicate patient progress, barriers, and treatment plans with supervisors and healthcare team members.
  • Complete discharge summaries.
  • Maintain electronic documentation and patient records.
  • Accurately record treatment minutes.
  • Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and other required meetings.
  • Participate in facility/company trainings, in-services, and conferences.
  • Obtain necessary DME and supplies.
  • Supervise/support OT assistants, rehabilitation aides, and students.
