Key Responsibilities

• Evaluate and treat patients in a post-acute care setting to restore and improve functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to coordinate comprehensive patient care
• Educate patients and caregivers on therapeutic techniques and home exercise programs

Required Qualifications

• Active OT or COTA license in the state of California
• Current CPR/BLS certification
• Ability to work day shift hours
• Expected 35 hours per week

Preferred Experience / Skills

• Prior experience in a post-acute or skilled nursing facility setting
• Familiarity with documentation and EMR systems commonly used in post-acute care environments
• Strong communication and patient-centered care skills
