Job Title

Certified Occupational Therapy Assistant (COTA)

Location

Preston, MN | Skilled Nursing Facility / Home Health

Schedule / Job Type

Contract | Days | Monday through Friday, 7:00 AM–3:00 PM or 8:00 AM–4:00 PM (flexible) | Expected 32 hours/week

Key Responsibilities

  • Provide occupational therapy services to patients in both Skilled Nursing Facility (SNF) and Home Health settings
  • Meet productivity expectations of 87% in the SNF setting and 95% in the Home Health setting
  • Utilize Net Health and Home Care Home Base software for documentation and patient management
  • Deliver patient care under the supervision of an on-site Therapy Supervisor
  • Follow established care plans and contribute to patient progress toward functional goals

Required Qualifications

  • Active COTA licensure or certification in the state of Minnesota (or eligibility to obtain)
  • Must reside within 80 miles of the facility to be eligible for this position
  • Ability to work across SNF and Home Health care settings
  • Proficiency or ability to learn electronic documentation platforms

Preferred Experience / Skills

  • New graduates are welcome to apply
  • Prior experience in SNF or Home Health settings is a plus
  • Familiarity with Net Health and/or Home Care Home Base documentation software
