Perform focused physical assessments and develop plans of care based on diagnosis and individual client needs. Recommend medications and treatments as appropriate. Develop, revise, and evaluate standards of care. Assist in the selection of supplies, technology, and equipment. Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wound treatments. Evaluate the quality and delivery of care provided by clinical staff. Collaborate with the health care team and make referrals to other disciplines as indicated. Teach, counsel, and assist individuals and families in taking responsibility for prevention of complications related to wounds, ostomies, or continence problems. Develop, provide, and evaluate staff education programs such as new employee orientation and continuing education. Provide comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures. Communicate challenges identified, including ethical issues and conflict resolution. Collaborate with nursing leadership to recommend best practices based on prudent assessments. Participate in quality improvement activities. Provide culturally competent education consistent with cultural, ethnic, and religious belief systems. Maintain the security and confidentiality of data in accordance with patient rights policies. Achieve and maintain a level of professional competency through engagement in relevant clinical activities. Identify available community resources to assist clients and help them utilize these resources. Manage assets related to rented medical equipment.
