Job Description: • Coordinates all aspects of care (inpatient and/or outpatient) - per program assignment, including but not limited to record review, medication refills and/or prior authorizations; patient education, review of test/procedure/lab results
- Makes referral for medically necessary services, consults, equipment and supplies.
- Functions as a liaison between administrative, clinical staff and physicians within the transplant department, as well as other departments involved with the care of patients.
CARE PROVISION:
- Assists and supports patient discharge planning as needed
- Provides patient education regarding care and medications
- Provides assistance to patients and/or physicians in clinic and on hospital floors, as needed.
- Carries out patient care orders as directed by physician and in accordance with hospital policy.
- Assists with patient admissions and discharges as required
Requirements
BLS
CA License
Bachelor's
EPIC Phoenix (strict requirement)
Outpatient experience in EPIC.
Shift Details: M-F, 5x8s no weekends or holidays
