Job Responsibilities:
Assessment: Performs assessments and reassessments as warranted by patient condition.
Demonstrates sound independent decision-making ability.
Development and Implementation: Initiates, prioritizes, and implements the plan of care.
Collaborates with patients, families, and interdisciplinary services.
Utilizes resources appropriately to support the needs of the organization and patient.
Evaluation: Monitors and documents responses to clinical interventions.
Education: Facilitates and coordinates education specific to patient and family needs.
Leadership: Delegates tasks to licensed and non-licensed personnel.
Functions as a mentor and preceptor.
Encourages and facilitates teamwork.

Implements practice changes and complies with regulatory standards.
