Delivers patient care by incorporating principles of Relationship Centered Care. Utilizes the nursing process based on theory, research, evidence-based practice, and organizational nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes patient care including teaching, rounding, coaching, and planning after hospital care, delegating to others as appropriate. Educates patients and caregivers about their plan of care and promotes health and disease prevention. Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model and demonstrates effective allocation of resources. Role models principles through various patient care actions, including admissions, bedside shift reports, and purposeful rounding. Considers patients' values, preferences, and cultural diversity in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patient perceptions of care. Initiates service recovery for patients and families with concerns and escalates as needed. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff in using evidence-based practices to improve patient perceptions of care. Conducts purposeful hourly rounding addressing key patient needs. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety. Implements process improvement strategies based on evidence-based practices. Engages in formal and informal peer review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and research studies. Contributes to quality and safety practices and ensures compliance measures. Implements principles of high reliability to identify and avoid high-risk behaviors. Escalates potential safety hazards or gaps from best practice accordingly. Utilizes information technology for documentation to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with leadership to ensure proper utilization of supplies and inventory for patient care. Escalates resource deficits to management as necessary. Documents accurately and thoroughly in compliance with regulatory requirements and standards of care. Facilitates patient throughput during shifts. Contributes to a supportive environment for clinical colleagues and patients. Advances clinical competence in nursing practice. Projects a professional image and communicates with respect and empathy. Incorporates ethical principles into decision-making for patient and family care. Encourages lifelong learning and innovation for self and others. Acquires knowledge relevant to the role, patient population, and community needs.
