Comprehensive, multidisciplinary care is provided to individuals recovering from strokes, orthopedic surgeries, debilitating conditions, and brain injuries. The goal is to assist patients in regaining their highest level of independence and functionality. An interdisciplinary team, including physicians, nurses, therapists, and support staff, collaborates to develop individualized care plans. Family involvement and education are emphasized to support the patient's recovery journey. Advanced rehabilitation technologies and evidence-based practices are utilized to ensure the highest standard of care. Patient safety, dignity, and respect are prioritized in all aspects of care. Staffing and scheduling include self-scheduling options, with patient ratios of 1:7. Float to long-term acute care or other units may be required to manage med-surg patients. Weekend coverage is required, with a pre-approved time off process in place. The patient demographic includes adolescents, adults, and geriatric groups, with a daily census of 35. The facility is equipped with fully equipped therapy rooms and gyms, alongside state-of-the-art equipment for physical and occupational therapy. Patient and family education and support services are provided. Transitional care planning is offered for a smooth return to the community. Additional skills preferred include accessing/managing ports, blood product administration, central line blood draws, colostomy/ileostomy management, CVA care, feeding tube management, heparin and insulin administration, isolation precautions, and phlebotomy. Experience with surgical drains, total joint replacements, tracheostomy care, and traumatic brain injuries is also valued. The role involves use of rapid response and code teams, as well as wound care management.
