Job Title: RN - Step Down Profession: Registered Nurse Specialty: Step Down Duration: Long-Term Shift: AM Hours per Shift: 12 hours Experience: Minimum 18 months of recent cardiac unit experience License: Valid Registered Nurse license in the relevant state Certifications: Required competencies include the ability to interpret cardiac rhythms and pass a Dysrhythmia Exam on the first attempt Must-Have: Must have experience with cardiac drips including Insulin, Milrinone, Dobutamine, Amiodarone, Heparin, Lasix, Nitroglycerin. Must demonstrate chest tube experience. Must have a rating of "4" on all cardiac skills on the skills checklist. Experience with telemetry interpretation is essential. Heart transplant experience is preferred, with training available if needed. LVAD experience is preferred, with training available if needed. Description: These units focus on high-acuity, heavily cardiac-focused PCU care. Travelers should have a strong cardiac background. Nurse-to-patient ratios typically range from 1:4 to 1:5. Expect high patient acuity, frequent admissions and discharges, and significant procedural volume. Common patient populations include those with heart failure, atrial fibrillation, atrial flutter, supraventricular tachycardia, ventricular tachycardia, ventricular fibrillation, fluid volume overload, chest pain, hypertension, heart transplant patients, congenital heart disease, and LVAD patients. Common procedures performed include right and left heart catheterizations, TR Band management, sheath removal and management, stent placement, valve replacement procedures, STEMI/NSTEMI care, pacemakers, ICDs, cardioversions, and ablations. Travelers may float to similar cardiac or PCU units as needed. Travelers will not float to the Cardiac Monitor Tech role. Only full-time staff nurses are eligible to float into Monitor Tech positions. Ensure comfort with cardiac acuity, telemetry requirements, and procedures before accepting this assignment.
