Job Responsibilities: Assessment: Performs assessments and reassessments per policy and procedure or as warranted by patient condition. Demonstrates the ability to make sound independent decisions. Development & Implementation: Initiates, prioritizes, and implements the plan of care. Reviews and revises the plan as indicated. Collaborates with patients/families and interdisciplinary services. Utilizes resources appropriately to support the needs of the organization, patients, and families. Evaluation: Monitors and documents responses to clinical interventions.
