A problem list is developed, prioritizing patient needs based on inter-professional team input. The RN fully implements patient care therapies and procedures in accordance with established guidelines. The RN explains procedures, treatments, and processes to patients and their families. Initiatives are taken to alleviate patient stress and discomfort while recognizing, responding to, and communicating changes in patient conditions appropriately. Ongoing assessments and responses to procedures are documented according to hospital policies and established standards. The RN delegates tasks appropriately and oversees team members in patient care activities. Accuracy and completeness of delegated care are monitored in implementing the plan of care. Educational needs of patients are identified and addressed related to prioritized problems based on comprehension level. The discharge process is facilitated by executing discharge orders in a timely manner, with appropriate teaching and coordination with case management.
