Job Responsibilities: Assessment: Performs assessments and reassessments per policy or as warranted by patient condition. Demonstrates the ability to make sound independent decisions. Development and Implementation: Initiates, prioritizes, and implements the plan of care and reviews and revises the plan as indicated. Collaborates with patients, families, and interdisciplinary teams. Utilizes resources effectively to support the needs of patients and families.
