Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment dedicated to healing and wellness. Nursing practices are based on evidence, current research, and established policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and other disciplines. Responsibilities include direct patient care activities such as assessments and treatments. Facilitates admission, transfer, and discharge activities. Provides patient and family-centered education and care coordination. Conducts comprehensive assessments in accordance with applicable nursing standards. Identifies patient risk factors and presenting complaints effectively. Develops a problem list, prioritizing patient needs based on input from the healthcare team. Designs actions and interventions relevant to the patient’s culture and age. Determines desired outcomes in coordination with patients and families, in accordance with the medical plan of care. Explains procedures, treatments, and processes to patients and families. Initiates action to alleviate patient stress and discomfort. Recognizes and communicates changes in patient conditions appropriately. Conducts ongoing assessments and documents responses to therapies. Delegates tasks appropriately and oversees team members in patient care activities. Monitors accuracy and completeness of delegated patient care. Identifies educational needs of patients related to prioritized problems. Facilitates the discharge process by executing discharge orders timely. Provides input into patient/nurse assignments considering staffing, activity, and patient acuity. Demonstrates active participation in unit-based activities supporting organizational values. Promotes a positive group dynamic and healing environment. Assures nursing excellence according to accepted standards and evidence-based practices. Participates in quality improvement activities and core measures. Uses a holistic perspective regarding patient care.
