Contract | Full-Time | Days | Monday – Friday, 8-Hour Shifts | 14-Week Contract
Key Responsibilities
- Provide skilled home health nursing care to patients across an assigned county territory, with a primary focus on the Shelton area and surrounding towns including Ansonia, Derby, Seymour, and Oxford (New Haven County)
- Perform comprehensive patient assessments and deliver care in accordance with individualized care plans
- Provide and manage wound care as a core clinical responsibility
- Document patient visits and clinical notes using the PointCare EMR system
- Meet productivity expectations as a Primary Care Nurse, with an expected output of 28 units per week within a 40-hour work week
- Complete orientation in the first week with an expectation of full productivity by the second week
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license in the state of Connecticut
- Prior home health nursing experience required
- Wound care experience required
- Ability to work independently in a field-based, home visit environment
- Fingerprinting is required for placement
- No on-call requirement
Preferred Experience / Skills
- Familiarity with PointCare or similar home health EMR platforms
- Experience managing a caseload across multiple towns or geographic territories
- Strong time management and self-directed work habits suited to a field-based role
