Based on assessment including potential risk, demonstrates the ability to develop a plan of care with goals and outcomes. Incorporates therapeutic relationships with patients and their families in care delivery. Acts as a liaison between physicians, patients and families, and collaborates with other health care members. May participate in health center and department committees. Takes formal responsibility for teaching and professional development of others. Implements and evaluates the plan of care based on identified patient goals and intended outcomes. Modifies the plan of care based on reassessment of the patient’s status. Completes documentation according to established standards and utilizes established documentation tools appropriately. Assesses the readiness of the patient and significant others to learn and implements the individualized education plan according to their needs. Leads in departmental or unit-based initiatives, including providing in-service opportunities or serving as project point for committees and task forces. Provides age-appropriate care for specific populations served. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Ensures an excellent service experience by consistently demonstrating established standards of performance each day.
