Description: The RN implements professional nursing practice centered on the patient as a whole person. Nursing practices focus on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence, current research, hospital policies, regulatory standards, and professional guidelines. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaboration among nursing staff and across other disciplines. Responsibilities include: - Performing and documenting comprehensive patient assessments according to scope of practice and hospital policies. - Considering the patient’s culture, age, and developmental level in assessments. - Identifying patient risk factors and presenting complaints. - Collecting objective and subjective data. - Verifying initial admission assessments. - Developing problem lists and prioritizing patient needs based on inter-professional input. - Designing actions and interventions relevant to the patient’s context. - Determining desired outcomes in coordination with patients and families. - Implementing patient care therapies and procedures within established policies and guidelines. - Explaining procedures and treatments to patients and families. - Initiating actions to alleviate patient stress and discomfort. - Recognizing and communicating changes in patient conditions. - Documenting ongoing assessments and responses to treatments. - Delegating tasks appropriately and overseeing team members. - Monitoring accuracy and completeness of delegated patient care. - Identifying and addressing educational needs of patients. - Facilitating timely patient discharge processes. - Providing input into patient/nurse assignments based on staffing and patient acuity. - Actively participating in unit-based activities and promoting organizational values. - Creating a positive and healing environment through supportive interactions. - Contributing to quality improvement activities and core measures. - Using a holistic perspective regarding patient care.
