Description:
This is a Triage position in a busy Hematology/Oncology clinic with an infusion suite.
The average patient ratio is 1:6.
Responsibilities include administration of ordered injections, pump disconnects, and vaccines.
The role involves triaging telephone calls from patients, families, and referring offices.
Follow-up calls to patients as instructed by the provider are essential.
Prescription refills and home care referrals are part of the duties.
Collaborate closely with providers and nursing staff.
Review and triage lab and x-ray results received in the office.
Notify the medical practitioner of critical values.
Making follow-up calls to high-risk patients may involve assessments of changes in status.
Document information provided to patients during triage in electronic health record systems.
Communicate with physicians any additional documentation or notes from triage events.
Perform patient vitals, medication reconciliation, health histories, and document in electronic medical records.
Conduct pre-visit reviews to ensure availability of required information for providers during visits.
Evaluate patients' progress toward clinical and educational outcomes.
Perform in-office procedures such as EKGs, PFTs, and assist with bone marrows and surgical procedures as ordered by providers.
Conduct Nurse Only visits for patients not requiring a physician or NP.
These visits may include blood pressure checks, immunizations, injections, and education.
