Patient-family centered care principles are incorporated into practice. The Clinical Nurse provides quality patient care under the direction of the Nurse Leader. Essential functions include applying the nursing process of assessment, planning, implementation, evaluation, and discharge needs of patients and families. Core Concepts of Patient- and Family-Centered Care (PFCC) are utilized to personalize care. The impact of nursing care and patient/family partnerships on patient outcomes is recognized. Collaboration with patient/family input and goals assists in planning care. Quality care is provided that meets or exceeds expectations regarding core measures, quality metrics, and patient experience. Assistance is given to individuals considering their special needs, age-related, and cultural-related requirements. Recognition of the roles of the interdisciplinary team and care partners is essential in planning patient care. Delegation of appropriate tasks is demonstrated. Knowledge of and competence in performing technical procedures and proper equipment use are necessary. Timely and accurate documentation in the patient record is required. Development of teaching strategies for patients and families is expected, along with appropriate documentation of education and learning. Leadership in the professional practice setting is demonstrated. Continuous personal education, mentoring of peers, and educating patients and families are key responsibilities. Staying up to date with current health system initiatives and incorporating evidence-based practice is important. Knowledge in the area of clinical practice must be maintained. Completion of self-appraisals including professional goals annually and peer reviews is expected. Participation in department-based activities, staff meetings, and committees is required. Engagement in handoffs of care involving patients, families, and partners in care is necessary. Cultural traditions and lore of the community served must be understood and embraced. Qualifications require an Associate's Degree or equivalent from an accredited school of nursing. A Bachelor's Degree is preferred. Registered Nurse license from the state is required. Various specialty certifications are necessary within specified timeframes. Physical demands include the ability to handle and carry weights as specified.
