Develops a plan that prescribes interventions to achieve desired outcomes. Implements the care plan and prepares for health promotion and a safe environment. Evaluates progress toward achieving patient care outcomes. Collaborates with the patient, family, and healthcare providers to ensure quality care. Provides learning opportunities for patients and family members. Directly provides health information to patients and treatment teams. Participates in discharge planning to ensure continuity of care.
