Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that prioritizes healing and wellness. Nursing practices are based on evidence and current research as well as department policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. The role promotes nursing excellence through helping relationships among nursing staff and across other disciplines. Coordinates, ensures, and provides direct patient care activities. This includes conducting nursing rounds, assessments, and treatments, and facilitating admission, transfer, and discharge activities. Provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. Performs and documents comprehensive assessments in accordance with the nursing scope of practice and hospital policies. Considers the patient’s culture and age in assessment. Identifies patient risk factors and presenting complaints. Obtains patient health information from family or others when necessary. Collects objective and subjective data. Verifies initial admission assessments if obtained by other healthcare team members. Develops a problem list and prioritizes patient needs based on input from the inter-professional team as appropriate. Designs actions and interventions relevant to the patient’s culture and age. Determines desired outcomes in coordination with the patient and family according to the medical plan of care. Fully implements patient care therapies and procedures while adhering to established policies and professional guidelines. Explains procedures, treatments, and processes to patients and families. Initiates action to alleviate patient stress and discomfort. Recognizes, responds to, and communicates changes in patient conditions appropriately. Conducts and documents ongoing assessments and responses to procedures and therapies. Sets priorities for the care of multiple patients. Assures resources and coordinates the plan of care. Organizes workload for effective utilization of time and energy relative to patient acuity. Delegates tasks appropriately and oversees team members in the performance of activities regarding patient care. Monitors the accuracy and completeness of delegated patient care provided by other team members. Identifies and addresses educational needs of patients related to prioritized problems according to their level of comprehension. Facilitates the patient discharge process by executing discharge orders in a timely manner. Provides input into patient/nurse assignments considering available staffing and patient acuity. Demonstrates active participation in unit-based activities that support organizational values and priorities. Promotes a positive group dynamic and healing environment through words and actions. Assures nursing excellence in accordance with accepted standards and evidence-based practices. Actively participates in quality improvement activities and core measures.
