Job Opportunity – Registered Nurse, Case Management & Discharge Planning – Arizona
Start Date: October 6, 2026
Location: Arizona
Compensation: Details provided to qualified applicants
Schedule: 8-hour shifts, Monday through Friday, 8:00 a.m. to 5:00 p.m. Staff meetings required as scheduled.
Highlights:
- Provide case management and discharge planning for age-appropriate inpatients with chronic diseases and special healthcare needs
- Work across multiple units including Multi-Service, Intensive Care, OB, and Adolescent Care
- Coordinate admissions and transfers with other healthcare facilities through comprehensive tracking systems
- Perform thorough chart reviews to facilitate safe return to facility or discharge home
- Predictable weekday schedule with no weekend or holiday requirements
- New Employee Orientation provided every other Monday
Requirements:
- Valid and unrestricted Registered Nurse license in any U.S. State or Territory
- Minimum two years of discharge planning experience required
- Medical-Surgical nursing experience required
- Clinical experience in acute care and/or critical care settings
- Valid American Heart Association Basic Life Support (BLS) certification with commitment to maintain
- Must be fully cleared 7 days prior to start date
