Supervises and directs the activities of various levels of assigned nursing staff. Coordinates care with other disciplines while utilizing critical thinking, professional discretion, and independent judgment. Collects relevant data pertinent to the patients' health or situation. Analyzes assessment data to determine diagnosis and care issues. Develops a care plan that prescribes interventions to achieve desired outcomes. Implements the plan, coordinates care delivery, and employs strategies to promote health and a safe environment. Evaluates progress toward attaining outcomes. Identifies expected outcomes for the patient or the patient's situation. Collaborates with the patient, family, and healthcare providers in providing safe and humane patient care. Provides learning opportunities for patients and team members. Directly provides health information to patients and families. Participates in discharge planning to ensure continuity of care.
