Description: The RN implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research, as well as department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities through the provision of nursing rounds, assessments, and treatments. This role involves facilitating admission, transfer, and discharge activities, as well as providing patient and family-centered education. Care coordination and individualized treatment planning are also essential components of this role. The RN performs and documents a comprehensive assessment in accordance with their scope of practice and policies and procedures. Consideration of the patient's culture, age, and developmental level is crucial. The RN identifies patient risk factors and presenting complaints and obtains health information from family or others as necessary. They collect both objective/quantitative and subjective/qualitative data. The RN verifies initial admission assessments if obtained by other members of the healthcare team. The RN develops a problem list, prioritizing patient needs based on input from the inter-professional team as appropriate. Designed actions and interventions consider the patient's culture and age. Determining desired outcomes in coordination with the patient and family is an ongoing process. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy within their scope of practice. They explain procedures, treatments, and processes to patients and families. Initiating action to alleviate patient stress and discomfort is a priority. Recognizing, responding to, and communicating changes in patient condition appropriately is essential. Ongoing assessments and responses to procedures and therapies are documented according to policy, established standards, culture, and the age of the patient served. The RN delegates tasks appropriately and oversees team members in the performance of activities regarding patient care. Monitoring the accuracy and completeness of delegated patient care provided by team members is critical in the implementation of care plans. Identifying and addressing educational needs of patients related to prioritized problems is necessary based on the patient's level of comprehension. The RN facilitates the patient discharge process by executing discharge orders timely and ensuring discharge teaching is performed. They provide input on patient/nurse assignments considering available staffing, unit activity, and patient acuity. Demonstrating active participation in unit-based activities in support of organizational values and priorities is expected. Promoting a positive group dynamic and healing environment through words and actions is vital. Assuring nursing excellence in accordance with accepted standards, evidence-based practice, current research, and a caring philosophy is required. The RN actively participates in department and unit quality improvement activities and core measures related to patient satisfaction, safety, and care quality. Using a holistic perspective regarding patient care is essential.
