Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence, current research, hospital policies, and regulatory standards. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across disciplines. Responsibilities include assessment of patient needs, performing comprehensive assessments, and documenting findings. Nurses will consider the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is essential. Obtaining health information from family or others as necessary is part of the role. Objective and subjective data collection is required. Verification of initial admission assessments must be completed. A problem list must be developed, prioritizing patient needs appropriately. Designing actions relevant to the patient’s culture and age is expected. Desired outcomes should be determined in coordination with the patient and family. Implementing patient care therapies and procedures must align with established policies and procedures. Explaining procedures, treatments, and processes to patients and families is required. Taking action to alleviate patient stress and discomfort is necessary. Recognizing, responding to, and communicating changes in patient condition is essential. Ongoing assessment and response to procedures and therapies must be conducted and documented. Delegating tasks appropriately and overseeing team members in patient care activities is required. Monitoring the accuracy and completeness of patient care is part of the role. Addressing educational needs of patients related to prioritized problems is necessary. Facilitating the patient discharge process and executing discharge orders in a timely manner is required. Providing input into patient/nurse assignments is expected. Active participation in unit-based activities supports organizational values and priorities. Promoting a positive group dynamic and healing environment is essential. Nursing excellence must be assured in accordance with accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is required. Using a holistic perspective in patient care is essential.
