The RN performs and documents a comprehensive assessment in accordance with scope of practice and policies/procedures. The RN considers the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is crucial. The RN collects objective and subjective data while verifying the initial admission assessment if obtained by other members of the healthcare team. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions relevant to the patient’s culture and age are designed. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to patients and families. The RN initiates action to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies are conducted and documented using the nursing process. Tasks are delegated appropriately, and team members' performance regarding patient care is overseen. The RN monitors the accuracy and completeness of delegated patient care provided by team members in the implementation of the plan of care. Educational needs of patients related to prioritized problems according to their level of comprehension are addressed. The RN facilitates the discharge process by executing discharge orders in a timely manner, identifying potential discharges, and coordinating with Case Management and others as indicated.
