The role involves creating a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as hospital policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities on the unit. This involves provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. Provision of patient and family-centered education is essential. Care coordination and individualized treatment planning are applicable as needed. Assessment of patient needs is a key function. The RN performs and documents comprehensive assessments in accordance with their scope of practice and hospital policies. Consideration of the patient’s culture, age, and developmental level is necessary. Identifying patient risk factors and presenting complaints is required. Gathering patient health information from family or others is necessary as needed. Collect both objective and subjective data. Verify initial admission assessments obtained by other team members. Develop a problem list, prioritizing patient needs based on input from the inter-professional team. Design actions and interventions relevant to the patient’s culture and age. Determine desired outcomes in coordination with the patient and family. Fully implement patient care therapies and procedures with empowerment and clinical autonomy. Explain procedures, treatments, and processes to the patient and family. Initiate actions to alleviate patient stress and discomfort. Recognize, respond to, and communicate changes in patient condition appropriately. Conduct ongoing assessments and document responses to procedures and therapies. Delegate tasks appropriately and oversee team members in patient care activities. Monitor the accuracy and completeness of delegated patient care provided by other team members. Identify and address educational needs of patients related to prioritized problems. Facilitate the patient discharge process by executing discharge orders in a timely manner. Provide input into patient/nurse assignments based on available staffing and patient acuity. Demonstrate active participation in unit-based activities supportive of organizational values. Promote a positive group dynamic and healing environment through words and actions. Assure nursing excellence in accordance with accepted standards and evidence-based practice. Actively participate in department and unit quality improvement activities and core measures. Use a holistic perspective regarding patient care.
