Care coordination with interdisciplinary teams
Description:
The RN will work on a 35-bed unit with a patient census of 35.
Patient types include orthopedic and trauma cases, post-operative surgeries, and patients with acute or chronic diseases.
Common diagnoses include fractures, head trauma, infections, and co-morbidities such as diabetes and hypertension.
Patient ratios are set at 1 RN to 4-6 patients, with LPN and CNA support.
The role involves discharge planning and patient education, particularly regarding renal and fluid management.
CNA duties include assisting with vital signs, glucose checks, bathing, feeding, and ambulating patients.
The unit has a resource team that floats across all units.
Telemetry monitoring is conducted in a central telemetry unit.
The hospital has a 24/7 respiratory therapy and hospitalist coverage.
Technology and equipment include an EMR system, IV pumps, cardiac telemetry monitors, and portable vital signs monitors.
Floating may occur to all adult med-surg and telemetry units in-house.
Schedule includes weekend rotation expectations and holiday shifts.
