Job Requirements

• Active Oregon and Washington RN license, or an active compact RN license on file
• Minimum of 1 year of case management or patient care coordination experience
• Current BLS certification
• Experience with Epic charting preferred
• Experience working with Medicaid and Medicare populations
• Strong communication, organization, and care coordination skills

Responsibilities

• Coordinate patient care needs across the outpatient specialty care setting
• Support scheduling, follow-up, and continuity of care for assigned patients
• Collaborate with clinical teams to help ensure timely, efficient care delivery
• Document accurately in Epic and maintain clear, up-to-date patient records
• Assist patients and families with care navigation and resource coordination
• Work with diverse patient populations in a fast-paced, team-oriented environment
• Support weekend and holiday coverage as needed
