Job Requirements

  • Active RN license in Oregon and Washington required
  • Minimum of 1 year of skilled home health nursing experience
  • BLS certification required
  • Oasis experience required
  • Experience documenting in Epic preferred and/or required
  • Primary series COVID vaccination plus one booster required
  • Strong communication, time management, and independent clinical decision-making skills
  • Ability to travel throughout the East Portland area and surrounding communities

Responsibilities

  • Provide skilled home health nursing care in patients' homes
  • Complete start of care, admission, recertification, resumption of care, routine visits, and discharge assessments
  • Perform wound care, wound vac management, medication assessments, and disease management support
  • Care for patients with PICC lines, G-tubes, ostomy care needs, and catheter care needs
  • Support medication administration, IV therapy, phlebotomy, and infection control precautions as needed
  • Assess pain, safety, and compliance while educating patients and families on care plans
  • Maintain accurate documentation in Epic, Rover, and Oasis workflows
  • Manage a daily schedule that may include multiple visits while keeping patient care organized and efficient
