Registered Nurse Job Requirements

• Active Registered Nurse (RN) license
• At least 1 year of recent home health RN experience
• Comfort working independently in a home care environment
• Proficiency with HCHB (Homecare Homebase) required
• Strong time management, clinical assessment, and documentation skills
• Ability to meet productivity expectations and complete documentation promptly

Responsibilities

• Provide skilled nursing care to patients in the home setting
• Complete assessments, develop care plans, and monitor patient progress
• Coordinate with the interdisciplinary care team to support safe, effective patient outcomes
• Deliver patient education and reinforce treatment plans
• Maintain timely, accurate documentation in HCHB
• Meet a minimum productivity expectation of 30 points per week
• Complete documentation by 10:00 a.m. the following day
• Travel within assigned counties as needed to provide patient care
