Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license eligible for practice in Connecticut
• At least 1 year of professional nursing experience; home health nursing experience is preferred
• Associate degree in Nursing or higher
• Current professional nursing credentials and certifications required for home health practice
• Ability to travel throughout the assigned service area and provide skilled nursing care in patients’ homes
• Strong communication, organization, clinical judgment, and electronic or clinical documentation skills

Travel RN Home Health Responsibilities

• Conduct comprehensive home health visits and provide skilled nursing care based on individualized patient needs and established care plans
• Assess patient conditions, monitor progress, identify changes in status, and communicate concerns promptly
• Provide patient and caregiver education to promote safe care, treatment adherence, and improved health outcomes
• Complete accurate, timely, and thorough clinical documentation for every home health visit
• Coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary healthcare team
• Manage a productive caseload while meeting an expected productivity level of approximately 30 points
• Deliver compassionate, patient-centered nursing care across Waterbury, Wallingford, and surrounding Connecticut communities
