Job Requirements

• Active Massachusetts RN license required
• Minimum of 1 year of nursing experience preferred, especially in home health, home care, or community-based nursing
• Associate degree required
• Ability to work independently and manage a patient caseload efficiently
• Strong clinical assessment, care planning, and documentation skills
• Comfortable working with post-acute care and assisted living patient populations
• Reliable transportation for travel between patient visits; mileage reimbursement applies between visits
• Professional, business-casual appearance required

Responsibilities

• Initiate, develop, implement, and revise patient plans of care in collaboration with the physician and interdisciplinary care team
• Provide direct and indirect supervision of home health aides and licensed practical/vocational nurses as appropriate
• Complete supervisory visits and ensure care aligns with state and federal home health regulations
• Deliver high-quality skilled nursing services within scope of practice and infection control standards
• Perform comprehensive patient assessments, including OASIS evaluations and medication reconciliation
• Document patient visits accurately and on time according to policy, payer, and compliance requirements
• Support safe, coordinated home health care for patients across Boston, Needham Heights, and surrounding service areas
