Home Health Registered Nurse (RN) Case Manager Travel Contract – Holland, OH / Toledo Area

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) Case Manager for a travel nursing contract in Holland, Ohio, serving patients throughout Toledo and greater northwest Ohio. Provide skilled, patient-centered home health care, coordinate individualized plans of care, and help patients and families achieve their health goals while enjoying a Monday–Friday schedule and guaranteed hours.

Home Health RN Travel Contract Details

• Location: Holland, Ohio, with coverage across Toledo, Fostoria, Findlay, and surrounding counties
• Healthcare Setting: Home health and community-based care
• Assignment Type: Travel nursing contract
• Assignment Dates: November 16, 2026 – February 12, 2027
• Contract Duration: 13 weeks
• Schedule: Monday–Friday, approximately 8:00 a.m.–4:30 p.m.
• Shift Duration: 8-hour shifts
• Hours: 40 hours per week with guaranteed hours
• Weekend/On-Call Requirement: One weekend and weekday coverage period may be required during the assignment
• Weekly Estimated Pay: $2,120–$2,230
• Travel Area: Mileage between patient visits is reimbursed according to assignment guidelines

Home Health Registered Nurse Job Requirements

• Current, unrestricted Registered Nurse (RN) license in the state of Ohio
• At least 1 year of professional nursing experience; home health nursing experience is strongly preferred
• Associate degree in Nursing or higher
• Demonstrated knowledge of OASIS assessments and home health documentation
• Strong assessment, clinical judgment, communication, and care coordination skills
• Ability to travel independently between patient visits throughout the assigned service area
• Homecare Homebase (HCHB) experience is preferred
• Professional certification or credentials required for practice, as applicable

Home Health RN Case Manager Responsibilities

• Serve as the primary nurse case manager for assigned home health patients
• Perform comprehensive nursing assessments, including OASIS assessments, in the patient’s home
• Develop, implement, evaluate, and update individualized plans of care
• Provide skilled nursing interventions, patient education, medication guidance, and disease-management support
• Monitor patient progress, identify changes in condition, and communicate concerns to appropriate care team members and providers
• Coordinate services with physicians, therapists, aides, caregivers, and other members of the interdisciplinary team
• Complete timely, accurate clinical documentation and maintain compliance with home health standards
• Support safe transitions of care and help patients remain independent in their home environment
• Manage a daily field schedule while meeting productivity expectations of approximately 30 points
• Participate in scheduled weekend or weekday coverage as assigned
